29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about 06/19/2017 at the Ardmore Street Re-Roofing Project, employees were using pneumatic nail guns to install shingles to the roof deck and no safety glasses were in use.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 06/19/2017 at the Ardmore Street Re-Roofing Project, employees were using extension ladders in the front and rear of the building to access and descend to and from the upper main roof and these ladders did not extend at least 3 feet past the landing.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: On or about 06/19/2017 at the Ardmore Street Re-Roofing Project, employees were tying off to the D ring located at the waist line instead of the D ring located in the rear of the harness while working from the roof deck.
