29 CFR 1910.29(b)(10):  The employer did not ensure that guardrail systems used at hoist area, equipped with  a removable guardrail section, consisted of a top rail and midrail, and were placed across the access opening between guardrail sections when employees were not performing hoisting operations. The employer may use chains or gates instead of a removable guardrail section at hoist areas if the employer demonstrates the chains or gates provide a level of safety equivalent to guardrails.    (a) Production - On June 22, 2017, the employer did not ensure that an adequate guardrail system consisting of a top and midrail was placed across the access opening on the mixer for the cold tape line.    (b) Production - On June 22, 2017, the employer did not ensure that an adequate guardrail system consisting of a top and midrail was installed across the access opening on the mixer for slitter #2.    In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.
