29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Building #13, on or about June 22, 2017, two employees were working from the roof deck removing and replacing roof sheathing and shingles without any means of fall protection, which exposed employees up to 24-foot fall hazards.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks: a) At the job site, in back of the building, on or about June 22, 2017, and at times prior to, a JLG #050744 rough terrain forklift was left unattended with its forks raised to support a load of wood roof sheathing and shingles, resulting in a struck-by hazard. ANSI B56.1-1969, Section 603 E. states, When leaving a powered industrial truck unattended, load engaging means shall be fully lowered..."
