29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     On or about August 15, 2019 at 3355 Vista Circle, Macon, Georgia, two employees sheathing the roof using the  aluminum extension ladder to access the upper working level were exposed to a fall hazard of approximately ten (10) feet.  The ladder did not extend three (3) above the eaves and was not secured in place.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).    ON or about August 15, 2019 at 3355 Vista Circle, Macon, Georgia employees sheathing and shingling the roof approximately 12 feet above the lower level were exposed to a fall hazard when working with out fall protection.      The CHC Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1241771, citation number 01, item number 001 and was affirmed as a final order on (date), with respect to a workplace located at211 Lanier Drive, Statesboro, Georgia 30458
