29 CFR 1926.403(b)(1)(iii): Safety of equipment was not determined on the basis of electrical insulation.  At a jobsite located at 3501 N. Rangeline Road, Joplin, Missouri, employees were exposed to electric shock and burn hazards. Temporary wiring used to power a Milwaukee circular saw had damaged outer insulation at multiple locations along its length.

29 CFR 1926.451(a)(6): Scaffolds were not loaded in accordance with the design by a qualified person.  At a jobsite located at 3501 N. Rangeline Road, Joplin, Missouri, employees were exposed to a serious fall hazard. Employees working from a "Safety Work Platform" elevated 20 feet above the ground using a Genie powered industrial  truck did not follow the manufacturer's design in loading the platform as specified on the equipment label. Employees did not use the fall protection system specified.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  At a jobsite located at 3501N. Rangeline Road, Joplin, Missouri, employees were exposed to a serious fall hazard. Employees working from a "Safety Work Platform" elevated 20 feet above the ground using a Genie powered industrial  truck did not use fall protection as specified by the equipment label.
