29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, inspections were not conducted by the competent person prior to the start of work and as needed throughout the shift or inspections were not made after every rainstorm or other hazard increasing occurrence: a) Job site, 3632 California Avenue, Pittsburgh, PA: On or about June 29, 2017, during sewer line repair activities, the employer did not ensure that a competent person inspected an excavation determined to be up to approximately 9 feet 7 inches in depth, exposing an employee to hazards such as being caught in an excavation collapse. Concerns noted during the inspection included, but were not limited to, failing to take appropriate actions for water accumulation, not ensuring proper cave in protection was utilized in the excavation, not keeping previously removed materials at least 2 feet away from the edge of the excavation and failing to ensure that safe means of egress was in use at all times.
