29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.      A.)  Chivers Construction Company, Inc.; 2810 Mill Fair Road, Fairview, PA; bridge area - On or about 6/29/17, and at times prior thereto, a midrail was missing on two areas of the bridge area.  The top rails were in place but midrails were not installed.  Employees were exposed to a potential fall of approximately 20 feet.

29 CFR 1926.350(a)(10): Oxygen cylinders in storage were not separated from fuel-gas cylinder or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour:   A.)  Chivers Construction Company, Inc.; 2810 Mill Fair Road, Fairview, PA; near job trailer - On or about 6/29/17, and at times prior thereto, two oxygen cylinders and one propylene cylinder were stored together in the same cabinet.
