29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    a) 65 Terminal Drive, Plainview, NY: The Injection Molder machine (Model #H6-150 ARS / Serial #556618150S79) operated by employees had a metal quad box (front of equipment) in which one of the outlets contained a broken ground pin; on or about 6/30/17.    b) 65 Terminal Drive, Plainview, NY: The Injection Molder machine (Model #H6-150 ARS / Serial #556618150S79) operated by employees had electrical cords (rear of equipment) in which the wires were exposed; on or about 6/30/17.    Note: Note: Because abatement of this violation is already documented in the case file, the employer needs not  submit certification or documentation of this violation as normally required by 29 CFR 1903.19.
