29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety:     (a)   On or about June 15th, 2017 and at times prior employees were exposed to cave-in hazards while installing a fire hydrant line in a trench approximately nine feet deep without protective system in the vicinity of 102 15th Avenue North in Casselton, North Dakota. 29 CFR 1926.600(a)(6)(iv): A person was not designated to observe clearance of the equipment and give timely warning for all operations where it is difficult for the operator to maintain the desired clearance by visual means:   (a)   On or about June 15th, 2017 employees were exposed to electrical hazards after the operator of an excavator made contact with a 12.5 kv overhead power line in the vicinity of 102 15th Avenue North in Casselton, North Dakota.
