29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structure:    Construction expansion to a Target store; On or about July 7, 2017, and at times prior thereto, employees using the entrance and exit to access the building where there were scattered pieces of masonry bricks, broken sheetrock, wood stakes, debris, boards with nails, and left over material, exposing employees to slips, trips, and fall hazards.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:   Construction expansion to a Target store, east and north side; On or about July 7, 2017, employees were exposed to fall hazards to the ground below, while performing stucco activities from scaffold which did not have base plates or mud sills below the support posts.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  Construction expansion to a Target store; On or about October 28, 2015, employees were exposed to fall hazards to the paved ground below, while climbing the scaffold end frames and cross braces to access the scaffold platform to lay brick.
