29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     The employer is failing to ensure that all employees exposed to eye and face hazards from flying particles while working on a residential roof.  This most recently occurred on June 28, 2017 on the jobsite located at 516 Sherwood Dr, Gretna, NE 68028.  An employee was observed using pneumatic nail gun to install roofing material without any form of eye or face protection.

This most recently occurred on June 28, 2017 on the jobsite located at 516 Sherwood Dr, Gretna, NE 68028.  Employees were observed installing shingles on a newly constructed residence with a 4 in 12 sloped roof at heights greater than 6 feet above the ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     The employer is failing to ensure that each employee who might be exposed to fall hazards has been effectively trained on how to use and operate personal fall arrest systems.  This most recently occurred on June 28, 2017 on the jobsite located at 516 Sherwood Dr, Gretna, NE 68028.  Employees were observed installing roofing materials on a residence with a 4 in 12 steep sloped roof at heights greater than 6 feet above the ground below without utilizing any form of fall protection.
