29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:    a)  On July 10, 2016, at Vision Awardcraft in Celina, Ohio:   The employer did not ensure that employees were provided with adequate training on electrical safety-related work practices such as, but not limited to, the use of personal protective equipment and lock out tag out to protect against electrical hazards while working on or near energized or potentially energized electrical parts. Employees were exposed to 480 volts electrical hazards while performing troubleshooting and repair work on an Allied heating and cooling unit.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:     a)  On July 10, 2017, the employer did not ensure that employees troubleshooting a 480 volt Allied heating and cooling unit were protected from electrical shock and arc flash hazards.  Employees were not wearing personal protective equipment such as, but not limited to, hand protection while performing electrical work.

29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed:     On July 10, 2016, at Vision Awardcraft in Celina, Ohio:  the employer did not assure that a lock and tag was applied to the disconnecting means for the Allied heating and cooling unit when an employee removed a relay.  Employees were exposed to 480 volt electrical hazards when performing work on the unit.
