29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:    On or about June 2017, in the wash line area of the facility where employees were exposed to chemical burns and struck-by hazards when performing service and/or maintenance tasks on the wash line without having developed specific procedures for controlling hazardous energy.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.    On or about July 13, 2017 in the crane assembly and fabrication area where employees were exposed to struck-by hazards when operating a bench grinder without a tongue guard.
