29 CFR 1910.28(b)(8): For repair pits, service pits, or assembly pits that were less than 10 feet (3 m) deep when fall protection systems were not required, the employer did not comply with provisions (i)-(iii) of this section: (a) On or about July 29, 2017 and at times prior, employees were exposed to fall hazards in excess of 4 feet into grease pits in that oil pit bay three was left open with a safety net that did not cover the entire oil pit when not in use. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet"). Abatement Note: In addition to the provisions listed in this section, acceptable abatement includes the use of conventional fall protection (such as guardrails) or safety net systems used in accordance with 29 CFR 1926 Subpart M.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made: (a) On or about July 13, 2017, employees were exposed to electrical shock hazards in the shop area; (1) The air pressure monitor electrical cord outer jacket was damaged and was not removed from service before repairs were made rendering equipment safe. (2) The shop vacuum electrical cord outer jacket was damaged and was not removed from service before repairs were made rendering equipment safe. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
