On or about July 18, 2017, the employer did not ensure employees were protected while using the platform next to the slitter roll to access knives. Employees are required to use the platform to adjust the knives on the slitter roll and are exposed to falls of approximately 9 feet to the concrete below.

On or about July 18, 2017, the employer did not ensure employees were protected while using the two catwalks on the S3 Slitter machine. Employees are required to use the platforms for setup purposes and are exposed to falls of approximately 6 feet and 4ft 3 inches to the concrete below and into the machine.
