29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a.)     The employer did not ensure employees were protected while operating Carpet Roller (CR1). On February 14, 2020 an employee was attempting to tuck paper into the final roller on CR1 when the machine was unexpectedly energized, resulting in injuries requiring hospitalization.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:     a.)     On or about February 14, 2020 the employer did not ensure lock out devices were affixed to the energy isolating device by an authorized employee. Employees are required to perform setup activities and do not apply a lock to the disconnect for the CR1 carpet machine.    Greto Heartland Industrial Packaging, Inc. dba Heartland Specialty Converting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection number 1400762 citation number 2, item number 1 and was affirmed as a final order on October 15, 2019 and inspection number 1248141 citation number 1, item number 3 and was affirmed as a final order on September 19, 2017 with respect to a workplace located at 1025 Findlay Road, Lima, OH 45802.
