OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to falls and caught-in amputation hazards:    918 N. Porter St. in Streator, IL 61364, warehouse, silo no. 2: on or about August 2, 2017, employees assigned to rinse the silo's interior for product changeovers were elevated up to 13 feet by a forklift supporting a portable personnel-carrying work platform, where they would step off the platform to access a fixed work platform at the upper portion of the silo.  While on the elevating platform, employees were exposed to falls and caught-in hazards from the moving components of the mast while extending or lowering, likely to result in multiple traumatic injuries and other physical harm (fingers/hands amputation).     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Porter St. in Streator, IL 61364, warehouse, silo no. 2, powered industrial trucks (counter-balanced propane-gas powered forklifts), personnel-carrying work platform: on or about August 2, 2017, forklift operators were assigned to elevate personnel in an unapproved work platform (unique workplace condition) for connecting product transfer hoses and rinsing the silo's interior for product changeovers. Operators did not receive adequate initial training in formal topics and practical exercises pertaining to safely elevating personnel in the work platform while operating truck and its lifting controls. Employees elevated in the work platform were exposed to fall hazards up to 13 feet likely to result in serious injury, and other caught-in hazards from moving forklift parts (mast, lifting carriage) likely to result in fingers/hands amputation.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Work positions to access the roof, walk on running boards, and get samples were in the immediate proximity (within 4 - 12 inches) to energized, unguarded, and not insulated high voltage electric power line running parallel to and above railcars. If the lines are to be deenergized, arrangements shall be made with the person or organization that operates or controls the electric circuits involved to deenergize and ground them. If protective measures, such as guarding, isolating, or insulating, are provided, these precautions shall prevent employees from contacting such lines directly with any part of their body or indirectly through conductive materials, tools, or equipment:  12th Street Norfolk Southern rail yard, Streator, Illinois, West industrial rail spur, ACFX 59074 center flow covered hopper rail car - on or about July 17, 2017, unqualified employees assigned to climb vertical fixed ladders on parked railcars' to access rooftop hatches for taking samples of plastic pellets with a job-made tool were exposed to contact with a high voltage (12kV) electric power distribution line.
