29 CFR 1917.118(e)(1): Fixed ladders more than 20 feet (6.1 m) in height shall be provided with a cage, well, or ladder safety device. (a) On or about July 26, 2017, at the end of the pier prior to accessing the fixed ladder to the waters edge below, the employer failed to ensure the carabiner used as the ladder safety device was functioning properly. Employees were exposed to falls.
