Employees were exposed to the hazards of eye injury from flying objects.

Employees were exposed to potential impact injuries caused by falling materials and/or equipment.

Employees did not use fall protection and were exposed to approximately 11 feet fall hazard from the roof to the ground below.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  A)    Charles Tucker, conducting work at 6800 Ridgewood Trail, Toledo, OH 43617: On or about August 2, 2017, the employer did not ensure that straight ladders, used to access the roof of the one story residential building, were secured at their tops to a rigid support, and grasping devices, such as grab rails, were provided to assist employees in mounting and dismounting the ladders. The employees were exposed to approximately 11 feet fall hazards when using the ladders to access the working level in order to conduct roofing activities.

Employees were exposed to approximately 11 feet fall hazard from the roof to the ground below.    Charles Tucker was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection # 995758, Citation number 1, Item number 1, and was affirmed as a final order on 11/14/2014, with respect to a workplace located at 4994 Main Street, Sylvania, OH 43560.
