(a) Charles Tucker on or about September 13, 2017, at the address of 3070 Hollywyck Road, Maumee, OH, 43537: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:  a) Charles Tucker on or about September 13, 2017 at the address of 3070 Hollywyck Road, Maumee, Ohio, 43537: The employer did not ensure employees were protected from falls while installing new roofing materials (tar paper) to a residential type structure.  The employees were exposed to a potential 8 foot fall to the level below.           Charles Tucker was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 995758, citation number 1, item number 1 and was affirmed as a final order on November 14, 2014, with respect to a workplace located at 4994 Main Street, Sylvania, Ohio, 43560.  Charles Tuck was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1252231, citation number 2, item number 1 and was affirmed as a final order on October 05, 2017, with respect to a workplace located at 6800 Ridgewood Trail, Toledo, Ohio,43617.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a) Charles Tucker on September 13, 2017 at the address of 3070 Hollywyck Road, Maumee, Ohio, 43537: The employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations.   Charles Tucker was previously cited for a violation of this occupational safety and health standard 1926.503(a)(1), which was contained in OSHA inspection number 1252231 , citation number 1, item number 3 and was affirmed as a final order on October 05,2017, with respect to a workplace located at 6800 Ridgewood Trail, Toledo, Ohio, 43617.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access.  a) Charles Tucker on or about September 13, 2017 at the address of 3070 Hollywyck Road, Maumee, Ohio, 43537: The employer did not ensure the side rails of the 24 feet fiberglass extension ladder was extended at least three feet above the landing surface. (Extension Ladder was taken apart and used as half). One employee was exposed to falls while using the ladder to access the scaffold and roof.  Charles Tucker was previously cited for a violation of this occupational safety and health standard 1926.1053(b)(1), which was contained in OSHA inspection number 1252231, citation number 1 , item number 4 and was affirmed as a final order on October 05, 2017, with respect to a workplace located at 6800 Ridgewood Trail, Toledo, Ohio, 43617.
