29 CFR 1926.451(c)(2)(i): Footings where not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement: (a) Excellence in Siding, at 6693 6th St., Frederick, CO 80530: On and before July 27, 2017, the employer did not ensure that scaffolding in use by employees had adequate footing for the surface on which it was positioned. Employees were using a pump jack scaffold to perform siding on the east side of the house, and the pump jack scaffold was set on the open ground with no footing. The pump jack scaffold was subject to settling or displacement, and did not have an adequate stable support. This condition exposed employees using the scaffold to fall hazards.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet above lower level, was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) Excellence in Siding, at 6611 5th St., Frederick, CO 80530: On and before July 27, 2017, the employer did not ensure that the guardrails in use with the pump jack scaffold met the requirements of paragraph (g)(4) of this section. The pump jack scaffold was missing its endrails and midrails. This condition exposed employees using the scaffold to fall hazards of approximately 20 feet.

An employee was performing siding and soffit work while working from the roof and was not using fall protection. This condition exposed employees to fall hazards of approximately 20 feet.
