29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  a)  Blue Barn Meadows - On or about August 8, 2017, an employee was using a circular saw with a damaged flexible power cord which has the sheathing torn to a degree that exposed the internal conductors.  b)  Blue Barn Meadows - On or about August 8, 2017, an extension cord set was in use to power portable equipment with exposed internal conductors.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  Blue Barn Meadows - On or about August 8, 2017, an extension ladder being used to access the patio roof, approximately 10 feet above grade, did not extend at least three feet above the upper landing surface.
