29 CFR 1926.21(b)(2):     The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.    On or about August 11, 2017, at 6982 E. Appleton Circle, Centennial, CO, 80112, Ramos Roofing, a contractor working for Premier Roofing, LLC did not instruct employees in the recognition and avoidance of unsafe conditions and the regulations applicable to their work environment to control or eliminate any hazards or other exposure to illness or injury.

29 CFR 1926.28(a): The employer is responsible for requiring the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees.  On or about August 11, 2017, on a Premier Roofing LLC project, at 6982 E. Appleton Circle, Centennial, CO 80112, employees of Ramos Roofing LLC and/or its subcontractors, did not wear the appropriate PPE in all operations or at all times where there was an exposure to hazardous conditions.
