29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosure:      a)  Two electric panels in the basement were left without covers with exposed live parts.  Construction personnel were exposed to electric shock hazard when working nearby and when turning on and off light and power switches in the panels each day.        Location: 115 Kingston Ave, Brooklyn, N.Y.      On or about 8-9-2017      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.

Construction personnel were exposed to electric shock hazard operating ungrounded tools and extension cords.       Location: 115 Kingston Ave, Brooklyn, N.Y. 29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.     a)  Construction site power circuits (cables) were wired into the two uncovered electric panels in the basement without use of cable attachment fittings.  Electric wires can pull loose exposing construction personnel to electric shock hazard.     Location: 115 Kingston Ave, Brooklyn, N.Y.    On or about 8-9-2017    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
