29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket:  At 3127 California Avenue, St. Louis, MO 63118, employee(s) climbed over the standard guardrail system belonging to the Genie Aerial Lift to gain access to the roof of a church.  The fall distance to the concrete sidewalk below was approximately 25 feet.

29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  At 3127 California Avenue, St. Louis, MO 63118, employee(s) were removing pre-existing shingles from the roof of the church without any form of conventional fall protection in place.  The fall distance to the concrete sidewalk below was approximately 20 feet.  The slope of the roof was approximately 12:12.  This violation was observed on 08/17/17.
