29 CFR 1926.502(d)(6)(ii): Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged to each other;    On or about August 15, 2017 at the 2550 Clairville Road, Oshkosh Wisconsin jobsite; the Protecta Pro Series Shock Absorbing Lanyard had two snaphooks which were connected to each other as an employee was securing a ladder system in place exposing employee(s) to fall hazards.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.    On or about August 15, 2017 at the 2550 Clairville Road, Oshkosh Wisconsin jobsite; fall protection was not utilized while installing a section of caged ladder system exposing employees to an approximate 140 feet fall hazard.

29 CFR 1904.40(a): Basic requirement. When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours.  On or about September 12, 2017 the OSHA 300 Log of Work-Related Injuries and Illnesses and OSHA 300A Summary of Work-Related Injuries and Illnesses for 2014 to 2017 were not provided.  Information provided by the employer on October 13, 2017 did not include the requested report.
