29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a)     On or about August 18, 2017, and at times prior, the employer, KPB Printing, Inc., did not establish and implement an energy control program so as to protect employees who were exposed to the unexpected energizing, startup or release of stored energy while engaged in printing and associated operations.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  On or about August 18, 2017 and at times prior, the employer, KPB Printing, Inc., did not ensure employees, who were required to operate a Hans Muller Stitcher were protected from caught-in hazards due to exposed sprockets, chains and belts.  The in-feed belt guard on this machine was broken.      b)  On or about August 18, 2017 and at times prior, the employer, KPB Printing, Inc., did not ensure employees, who were required to operate a Stahl Folder (Model FB66B2) were protected from caught-in hazards due to exposed belts and pulleys. Two (2) standard guards available for this model were not in use that would have prevented exposure to caught-in hazards.    c)  On or about August 18, 2017 and at times prior, the employer, KPB Printing, Inc., did not ensure employees, who were required to operate a Polar Mohr paper shear were protected from potential crushing hazards due to movement of paper holding device before shearing action.  Employees were using their hands to align paper in conjunction with the action of the paper holding device.      d)  On or about August 18, 2017 and at times prior, the employer, KPB Printing, Inc., did not ensure employees, who were required to operate a Harris-Seybold Citation paper shear were protected from potential crushing hazards due to movement of paper holding device before shearing action.  Employees were using their hands to align paper in conjunction with the action of the paper holding device.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a)     On or about August 18, 2017, and at times prior, the employer, KPB Printing, Inc., had not developed and implemented a written hazard communication program to protect employees who are required to use hazardous chemicals including, but not limited to, glycol ether, naptha and acetone.       Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
