29 CFR 1904.39(a)(1): The employer did not report to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor, within 8-hours the death of an employee resulting from a work-related incident:   a. On or about August 18, 2017 at Jewel Foliage Rd and Jones Ave, Zellwood, FL, an employee was fatally injured on 08/18/2017 as a result of a work-related incident. The employer was aware of the fatal incident on August 18, 2017 at about 11:46 am and did not notify OSHA as required.
