29 CFR 1910.67(c)(2)(v):  A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift.  On or about August 24, 2017, at the east side of the commercial building, the employer did not ensure that an employee working from a vehicle mounted elevating and rotating work platform  was in use of fall protection.  The employee was exposed to a fall hazard greater than 6 feet.
