29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    On or about August 23, 2017, an employee conducting masonry operations on a residential property was exposed to a fall hazard of 14.3 feet when the employer failed to ensure that a ladder was provided as a means of access.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.    On or about August 23, 2017, an employee conducting masonry operations on a residential property was exposed to a fall hazard of 14.3 feet when the employer failed to ensure fall protection was utilized.    The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number  1184692,  citation number 2, item number 1 and was affirmed as a final order on February 23, 2016, with respect to a workplace located at 212 W. Saint Charles, Lombard, Illinois 60148.    The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number  1112298,  citation number 1, item number 2 and was affirmed as a final order on January 26, 2016, with respect to a workplace located at 2599 W. Higgins Rd., Hoffman Estates, Illinois 60169.    No abatement certification or documentation is required for this item.
