29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Southeast side: On or about September 01, 2017, employees without fall protection were exposed to fall hazards greater than 6 feet to the paved ground below, while removing old tiles on the roof of a commercial building.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder was not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  Southeast building:  On or about September 1, 2017, employees were exposed to fall hazards, while using a Little Giant extension ladder that was not set at a proper angle to gain access to a commercial roof.
