29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by 29 CFR 1910.178(l):     (a) Commercial construction, one employee operated a JLG 8042 forklift truck without having first completed training consisting of formal instruction, practical training, and an evaluation of the operator's performance in the workplace. Employees were exposed to struck-by and/or crushed by hazards.

(a) Commercial construction site, performing masonry work from tubular welded scaffold that was not restrained from displacement. Employees were exposed to scaffold collapse hazards.

29 CFR 1926.451(f)(12): Work on or from scaffolds is prohibited during storms or high winds unless a competent person has determined that it is safe for employees to be on the scaffold and those employees are protected by a personal fall arrest system or wind screens. Wind screens shall not be used unless the scaffold is secured against the anticipated wind forces imposed.    (a) Commercial construction site, employees were performing masonry operations on a 25-feet high tubular welded scaffold when wind screens were used and when the scaffold was not secured against the anticipated wind forces imposed. Employees were exposed to scaffold collapse hazards.
