29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.    Location:993 Belleville Turnpike, Kearny, NJ 07032-warehouse B    a) Employees exposed to fire and smoke inhalation hazards. Emergency exit was obstructed by 50 gallon drums and stocked metal materials on or about 9/7/17.

29 CFR 1910.252(a)(1)(i): Fire hazards. If the object to be welded or cut cannot readily be moved, all movable fire hazards in the vicinity shall be taken to a safe place.      Location:993 Belleville Turnpike, Kearny, NJ 07032-warehouse C    a) Employees exposed to fire and smoke inhalation hazards. The portable abrasive cutting wheel, used in welding operations, produced sparks near 50 gallon drums comprised of metal material and ammunition casing that contained gunpowder residue. Sparks ignited a small fire in warehouse B on or about 9/7/17.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards.    Location:993 Belleville Turnpike, Kearny, NJ 07032-warehouse S    a) Employees exposed to contact with flammable and combustible materials. Multiple cylinders comprised of Oxygen and Acetylene were stored near oil and other liquids on or about9/7/17.

If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing.      Location:993 Belleville Turnpike, Kearny, NJ 07032-warehouse A&B      a) Employees exposed to struck by and operating at unsafe speed hazards. Industrial vehicle operators performing functions at unsafe speeds and failed to slow down and signal other operators while traveling with and without loads on or about 9/7/17.     Metal green Recycling Industries was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178, which was contained in OSHA inspection number 1179118, citation number 1, item number 1 and was affirmed as a final order on 11/28/16, with respect to a workplace located at 993 Belleville Turnpike, Kearny, NJ 07032.
