29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   On or about September 11, 2017, during the installation of exterior surface material at 1407 N. Main St, N. Canton, Ohio 44720, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall and eye injury hazards where employees were working on a roof while installing exterior surface material with a fall height of approximately 10.5 feet.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about September 11, 2017, during the installation of exterior surface material at the job site located at 1407 N. Main St, N. Canton, Ohio 44720; the employer did not ensure employees working on the roof of a commercial location were protected from possible danger of eye injury from impact, or from flying objects from the employees installing exterior surface material.

On or about September 11, 2017; at a job site located at 1407 N. Main St, N. Canton, Ohio 44720; during commercial store front surfacing activities (installation of exterior tile), the employer did not ensure all employees using an aerial lift were protected from fall hazards of approximately 10.5 feet by a fall protection device.
