29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) On or about September 21, 2017, during the installation of roofing material at a job site located at 2114 North Ave Parma, Ohio 44134; the employer did not ensure employees working on a residential roof were protected from a fall of a height of approximately 14.0 feet on a 10:12 pitch roof by utilizing fall protection systems/devices.   b) On or about September 21, 2017, during the installation of roofing material at a job site located at 2114 North Ave Parma, Ohio 44134; the employer did not ensure employees working on a residential roof were protected from a fall of a height of approximately 10.0 feet on a 5:12 pitch roof by utilizing fall protection systems/devices.   c) On or about September 21, 2017, during the installation of roofing material at a job site located at 2114 North Ave Parma, Ohio 44134; the employer did not ensure employees working on a residential roof were protected from a fall of a height of approximately 10.0 feet on a 3:12 pitch roof by utilizing fall protection systems/devices.
