29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about September 22, 2017, employees removing shingles and laying felt on the roof of a residential home approximately 10.5 feet high with a 5:12 pitch were not protected from falling through the use of fall protection systems/devices.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On or about September 22, 2017, employees exposed to flying material and particulates while stripping shingles from the roof of a residential home were not wearing eye protection.     Sergey Prediy was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1195240, citation number 1, item number 1, and was affirmed as a final order on December 22, 2016, with respect to a workplace located at 6307 Snow Road, Parma, OH, 44129.
