29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On or about October 5, 2017, employees exposed to flying material and particulates while stripping shingles from the roof of a residential home were not wearing eye protection.     Sergey Prediy was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1195240, citation number 1, item number 1, and was affirmed as a final order on December 22, 2016, with respect to a workplace located at 6307 Snow Road, Parma, OH, 44129.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about October 5, 2017, during the installation of roofing material at a job site located at 17402 Laverne Ave. Cleveland, OH 44135; employees were climbing a portable ladder in order to access the roof and the employer did not ensure the ladder was not extended three feet above the landing surface. Employees were exposed to a fall height of approximately 10.8 feet.
