29 CFR 1926.352(i): Drums containers, or hollow structures which contained toxic or flammable substances were not, before welding, cutting, or heating was undertaken on them, either filled with water or thoroughly cleaned of such substances and ventilated and tested.   a) Plant - On or about 09/27/17 and at times prior thereto workers were instructed to perform electric welding on top of tank T-3003 without prior removal/control of flammable substances inside the tank to prevent ignition/explosion hazards.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) DSM Plant - On or about 09/27/17 the employer failed to ensure that provided fall protection was utilized to those workers who were exposed to a fall hazard.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
