29 CFR 1926.102(a)(1):   Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On 10/03/2017, employees from Allen Roofing & Siding were installing shingles on a roof of a residential building located on lot 105 on Underlay Court in Wentzville, MO.  Two employees were not wearing eye protection while using a pneumatic nail guns.

29 CFR 1926.501(b)(13):   "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 10/03/2017, employees from Allen Roofing & Siding were installing shingles on a roof of a residential building located on lot 105 on Underlay Court in Wentzville, MO.  Two employees were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The employees were observed on an 8/12 sloped roof of the building approximately sixteen to twenty-five feet above the ground without fall protection, exposing them to a fall to the ground below.

29 CFR 1926.1053(b)(16):   Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  On 10/03/2017, employees from Allen Roofing & Siding were installing shingles on a roof of a residential building located on lot 105 on Underlay Court in Wentzville, MO.  The employees were using a defective twenty-four foot portable extension ladder to access the roof.  The ladder had damaged or bent rungs and was missing or had illegible labels.
