29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  a)  At the establishment, on or about 10/3/17:  There were no periodic or annual inspections conducted of the energy control procedures (lockout/tagout) to ensure that the procedures and requirements of the standard were being followed.  Employees perform servicing and/or maintenance along with machine set up, changeovers, etc. on various equipment, including but not limited to CNC machines, saws, punches, press brakes, iron workers, etc.  Abatement certification must be submitted for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  Saw Dept., on or about 10/3/17:  Ingoing nip points on a chip auger for the Mastercut horizontal bandsaw, H-460-HA were not guarded.  b)  Sheet Metal Dept., on or about 10/3/17:  Point of operation on a Cincinnati 90 ton press brake was not guarded.  Abatement certification must be submitted for item b.
