29 CFR 1910.22(a)(3): (3) Walking-working surfaces  were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:   a)  Basement Stairs - Alley, Metal Stair Landing - On or about September 26, 2017, workers having to walk across a 5 foot by 5 foot metal stair landing were exposed to a 10 foot fall from the landing for the platform had its surface rusted through and several of its structural members which supported the platform were rusted completely through causing the platform to be structurally comprised.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces:   a)  Basement Stairs  On or about September 26, 2017, workers were exposed to a trip and 8 foot fall hazard where a 1 inch wide by 3 inch deep gap existed between a concrete floor slab and the first riser at the top of a set of wooden stairs.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.25(b)(3): Stairs did not have uniform riser heights and tread depths between landings:   a)  Basement Stairs  On or about September 26, 2017, stair risers were not uniform, in that, risers (from the top down) no. two was 7/34 inches high and riser no. three was 8 ¼ inches high where the remaining 8 risers were 9 inches high each.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements:   a)  Basement Stairs - On or about September 26, 2017, no handrials were provide where there was a set of wooden stairs which were 5 feet wide and having 10 risers thereby exposing workers to a 8 foot fall hazard.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
