29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.   a) LP Masonry Exteriors LLC on or about October 16, 2017 at the address of 6645 Airport HWY, Holland, OH: The employer failed to ensure an accident prevention program was in place to aid the employer and his employees in the recognition and avoidance of hazards associated with commercial masonry and fall hazards. (i.e.; fall protection, and use of personal protective equipment).

29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket while working from an aerial lift.   a) LP Masonry Exteriors LLC on or about October 16, 2017 at the address of 6645 Airport HWY, Holland, OH: The employer did not ensure employees were protected from a 28 foot fall hazard while working from a Genie S-40 Aerial Lift. The exposed employees were performing Stucco exterior operations from the aforementioned aerial lift.
