29 CFR 1910.269(q)(3)(xiv): The employer did not ensure that employees maintain the minimum approach distances, established by the employer under paragraph (l)(3)(i) of this section, from all grounded objects and from lines and equipment at a potential different from that to which the live-line barehand equipment is bonded, unless insulating guards cover such grounded objects and other lines and equipment:  Frontage Road near 20195 Hwy 281 N:  On or about October 24, 2017, employees were exposed to an electrical hazard while performing service work on an 34.5 kV primary power transmission line that was not adequately insulated.
