29 CFR  1926.251(d)(2): All splices in rope slings provided by the employer were not made in accordance with fiber rope manufacturers recommendations:  6416 Grissom Road, San Antonio, TX:  On or about August 18, 2022, and times prior thereto, employees were exposed to struck by hazards while using synthetic winch line with a splice that had not been made according to manufacturer's recommendations.

29 CFR  1926.959(a)(2): Inspection before use. The critical safety components of mechanical elevating and rotating equipment did not receive a thorough visual inspection before use on each shift.  (a) 6416 Grissom Road, San Antonio, TX:  On or about August 18, 2022, and times prior thereto, employees were exposed to struck by hazards while performing pole installation work with a digger derrick that had not been inspected for critical inspections, to include Hydraulic Overload Protection (HOP) inspections, in accordance with manufacturer's requirements.  (b) 6416 Grissom Road, San Antonio, TX:  On or about August 18, 2022, and times prior thereto, employees were exposed to struck by hazards performing pole installation work with a digger derrick that had not been inspected, including its safety devices, on a monthly and daily basis, in accordance with manufacturer's requirements.
