29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or likely to be present, which would necessitate the use of personal protective equipment.    a) 4200 Tremley Point Rd Linden, NJ 07036    At the time of the accident the employer failed to complete a job hazard analysis to determine which personal protective equipment and procedures would facilitate the task best.  The employer never performed a detailed hazard analysis for backing up equipment in the work yard.     Hazard occurred on 11/02/17

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a) 4200 Tremley Point Rd Linden, NJ 07036    The employer authorized employees to operate powered industrial equipment, which included, but not limited to a Kalmar Industries terminal tractor without providing powered industrial equipment training that included a combination of formal instruction, practical training, and a performance evaluation prior to operating terminal tractor. As per the manufacturers specifications this piece of equipment is not designed for use on a public road or roadway.       Condition occurred on 11/02/17
