(e) 1120 Zinn's Quarry Road, York PA (Building 4):  Both direct and temporary employees are sanitizing Hayssen machines without controlling the hazardous energy.  The employer did not ensure that the main service electrical panel and/or local disconnects were locked out, on or about November 15, 2017.    Bickel's Snack Foods, Inc.  was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1056244, citation number 1,  item number 1a and was affirmed as a final order on 10/6/2015 , with respect to a workplace located at 1120 Zinn's Quarry Road, York Pennsylvania.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.  ------------------------------------------------------------------------------------------------------------------------------    On 3/22/18 Informal Settlement Agreement modified Standard from 1910.147(c)(4)(i) to 147(d)(3)
