St Vincents Medical Center, located at 2213 Cherry Street in Toledo, Ohio; On or about November 6, 2017 the employer did not ensure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to employees cleaning the area under the dumpster after it is removed. Employees using shovels to scoop up materials and liquids that leak out of the dumpster are exposed to hazards associated with unknown substances that may contain infectious material.

St Vincent's Medical Center, located at 2213 Cherry Street in Toledo, Ohio; On or about November 6, 2017 the employer did not develop or implement procedures necessary for the safe permit space entry operations when entering the San-I-Pak, Inc. trash compactor, a decommissioned autoclave. Employees are exposed to crush hazards as they lean into the trash compactor to set pins on the inside of wall of the compactor.

A request was made on Febraury 1, 2018 for copies of the OSHA 300 logs for November 2017 and December 2017, as of April 18, 2018 no OSHA 300 logs were received.
