29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about November 16, 2017, employees were exposed to a fall hazard greater than 10 feet while installing roofing material on a residential home and fall protection was not used.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and  the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about November 16, 2017, employees were exposed to a fall hazard of approximately 10 feet while ascending and descending a ladder and the side rails did not extend at least three feet above the landing surface.
