29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:    (a) 2550 Cape Horn Road, Red Lion, Pennsylvania - The employer did not have a competent person involved in the job site in that none of the employees who worked on the site on November 16, 2017 inspected the job site for hazards and none had authority to correct hazards.  As a result, employees did residential roofing work without using any fall protection system, and employees used portable extension ladders that did not extend three feet above the landing surface to access the roof, exposing employees to the hazard of falling to a lower level, on or about November 16, 2017.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

S & A Contractors was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 915983, citation number 1, item number 1 and was affirmed as a final order on August 2, 2013, with respect to a workplace located at 2900 Whiteford Road, York, Pennsylvania  17401.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
