29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:    (a) 74 Clover Run Road Abbottstown , Pennsylvania - The employer did not have a competent person involved in the job site in that none of the employees who worked on the site on April 26, 2018 inspected the job site for hazards and none had authority to correct hazards.  As a result, employees did residential roofing work without using any fall protection system, and employees used portable extension ladders that did not extend three feet above the landing surface to access the roof, exposing employees to the hazard of falling to a lower level, on or about April 26, 2018.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) 74 Clover Run Road Abbottstown, Pennsylvania - At the front of the residential house, employees used a portable extension ladder to gain access to the roof to install the new roofing.  The ladder did not extended three feet above the roof landing surface, exposing employees to a fall of 13 to 14 feet to the ground below, on or about April  26, 2018.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    (a) 74 Clover Run Road Abbottstown , Pennsylvania - Employees worked installing new shingles on the single-family residential home that has a seven in twelve pitch.  The employees did not use any fall protection system, exposing the employees to the hazard of falling 13 to 14 feet to the ground below, on or about April 26, 2018.    S& A Contractors dba Dallastown Roofing was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 915983, citation number 1, item number 1 and was affirmed as a final order on August 2, 2013 with respect to a workplace located at 2900 Whiteford Road York Pennsylvania 17401.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
